Not every tooth can be saved with traditional root canal treatment. If inflammation persists at the tip of the root, microsurgery can help. At the Wurzelwerk dental practice in Kiel, we use modern microscope technology and minimally invasive procedures to preserve teeth even in difficult cases.

For surgical procedures such as root apex resections, we work closely with specialized practices and make targeted referrals if necessary. We carry out modern DVT diagnostics directly at our practice – for coordinated treatment.

When does microsurgical treatment make sense?

Microsurgical root canal treatment is used when

  • a previous root canal treatment was unsuccessful,
  • Inflammation at the root tip persists
  • anatomical features such as strongly curved roots,
  • conventional processing is no longer possible.

 

Why microsurgery?

Thanks to the finest instruments and high-resolution magnification, areas that are not visible to the naked eye can be reached. This leads to better chances of healing and long-term tooth preservation.

The process at a glance

After careful diagnostics with DVT and surgical microscope, the procedure is planned individually. Tissue-sparing access is made via the gums. The inflamed root tip is removed, the canal cleaned and closed with a retrograde filling. Wound healing is usually complete after a few days.

Gentle and targeted – microsurgery under visual control
We reach the inflamed area directly via a small access in the area of the root tip. We work with microscopic precision in order to spare healthy tissue.

Cleaning & sealing

Removal of the inflammation and bacteria-proof seal
The affected root tip is removed, the canal is cleaned and securely sealed with a biocompatible material (e.g. MTA) to prevent re-infection.

Control & aftercare

Long-term safety through close monitoring
Regular check-ups using X-rays or DVT ensure the success of the treatment and help us to optimally monitor the healing process.

Glossary microsurgical treatment – apicoectomy

 

A branch of dentistry that deals with the inside of the tooth – in particular with diseases of the dental nerve (pulp) and the root canal system.

A microsurgical procedure in which the tip of an inflamed tooth root is removed in order to preserve the tooth despite previous problems.

A bioactive material that is often used for bacteria-proof sealing of the root canal after surgical cleaning. It promotes the healing of the surrounding tissue.

Frequently asked questions about apicoectomy

When is an apicoectomy necessary?
Root tip resection is carried out if prior root canal treatment is not sufficient, e.g. in the case of inflammation at the root tip, cysts or fine canal branches that cannot be reached in any other way.
Is the procedure painful?

No – the procedure is performed under local anesthesia. It is usually less painful than feared.

How long does it take to heal?
The inflammation usually subsides within a few days. Bone healing takes around 6-12 months.
What is the success rate?
Thanks to the microscopic technique, the success rate is up to 90 % – especially when performed with practice.
